To obtain a treatment opinion, it is not always necessary to visit the clinic in person. With the aid of a completed pain questionnaire (www.pijnformulier.nl) and a copy of the MRI scan report, in most cases we can make a diagnosis and advise you of the likely treatment via the Internet.

This advice is free of charge.

Please complete the pain questionnaire and when prompted, enter the conclusion from the MRI scan report.

Alternatively, you may can print the form and fax it together with the MRI scan report to our fax number: +31(0)513 614861, or send it to us by post.

If instead of the MRI scan report you send a CD with the MRI scan, then you will be charged €50,62 for assessment of the images.
